Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards associated with lifts made from a
crawler type crane:
On or about 05/12/09 at Augusta, GA: Employees were exposed to struck by
hazards, while
performing concrete work at a construction site. The employer failed to
ensure
that the anti-two-
block device mounted on a Link LS 138H crawler crane was functioning
properly.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS
1. Instruct each operator on how to inspect the crane to ensure that the
anti-two-block device is
functioning properly prior to operation. Ensure that operators inspect
this as part of their regular
inspections of cranes.  (ANSI B30.5-2004, paragraph 5-1.9.9.1)
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
