Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to caught by hazards, while operating and maintaining press
brakes:
On or about August 28, 2009, at Universal Steel, 888 Buford Highway,
Buford, GA:Employees
were exposed to caught between hazards, while operating, performing
maintenance, and
changing dies on press brakes:
1.The employer failed to perform weekly press brake inspections and
maintain records of these
inspections.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
2.The employer failed to establish die setting procedures for press brakes.

AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THESE HAZARDS IS:
The employer should comply with the requirements of American National
Standard B11.3-1982.
Paragraph 5.1.4 directs employer to perform weekly inspections of press
brakes and maintain
records of these inspections.  Paragraph 6.1.5 directs employers to
institute general and special
purpose die design, construction, procurement and modification policies
and procedures that will
eliminate the need for the operator to place his hands or fingers within
the point of operation.

were not utilizing the forklift operator restraint system (seat belts).
exposing employees to
crushing injuries in the event that the forklift tipped over:
a)Production forklift operators were observed not wearing seat belts where
forklift trucks were
equipped with such safety devices on or about August 28, 2009.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to ensure that employees wear seat belts as required by ANSI B56.1
consensus standard for
powered industrial trucks.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
