Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to lower leg and foot injury: A.) On or about 2/2/10, at 215 Oneta Street, Athens Ga, employees were exposed to being struck by as they were required to use a job made lifting device at the 1-ton coffing hoist that has not been tested for its load. This hazard is recognized in safety documents: American National Standard, ASME B30.20a-1993 Below-The-Hook Lifting Devices. Among others, one feasible and useful abatement method to correct these hazards is to follow the ASME B30.20a-1993 Below-The-Hook Lifting Devices standard and the listed requirements in section 20-1.2.1a. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion hazards: On or about 2/2/10, at 215 Oneta Street, Athens, GA, employees were exposed to explosion and fire hazards as the system used to transport combustible carbon black dust was made of PVC piping which is a nonconductible material. The hazard is recognized in safety documents: NFPA 654, Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Solids, 2006 Edition, Section 9.3.1.1. Among other methods, one feasible and acceptable means of abatement would be to follow the requirements of NFPA 654, Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Solids, 2006 Edition, Section 9.3.1.1. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
