Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by or crushed-by steel tubing: (a)On or about 09/10/08, Saw Department, the 3 Ton P & H overhead crane had an inoperative safety latch. (b)On or about 09/10/08, Mill #2, the 3 Ton P & H overhead crane had an inoperative safety latch. Among other methods, one feasible and acceptable method to correct this hazard would be to install a safety latch on the hook in accordance with ANSI B30.10-1993, Section 10-1.2.3(c).

Among other methods, one feasible and acceptable method to correct this hazard is to require employees to utilize the seatbelts and reinforce this requirement during employee training.
