Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer
did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to the hazards of falls from elevation:
At the job site, located at 674 South Royal Oak Parkway, Fayetteville,
Arkansas,
employees were using wooden two job-made ladders without the filler blocks
inserted
between the rungs to support the cleats nor were the rungs recessed,
exposing the
employees to the hazards of falls from elevation.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to conform to the American National Standards Institute (ANSI)
A14.4.4.4-
1968 which states:  "Wood cleats shall be inset into side rails not less
than 1/2 inch or
shall be attached directly to the edge of the side rails, in which case
filler blocks of the
thickness of the cleats shall be securely attached to the edge of the rail
for the full length
between cleats, or equivalent construction"
