State Route 186 - On or about 10/04/08, the equipment operator who was
riding on the
back of the tailgate collecting the traffic cones lost his balance while
trying to keep them
from falling onto the ground after the foreman's small pick up truck hit a
bump or rock
in the road.
Abatement Note: Among many methods, one feasible and acceptable method to
correct the
following hazard is to continue to utilize the company's policy of walking
behind the truck while
retrieving or setting the traffic cones.c
