Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to death or serious injuries due to fall hazards from a
platform that could become
unstable.
(a) On 10/13/09, two employees were working from an elevated work platform
mounted on the
forks of the Pettibone rough terrain forklift.  The platform was 145
inches long, and the width
of the Pettibone was only 98 inches at the front outside wheel base.  This
design could result
in an unstable work platform.
Feasible means of abatement in this circumstance could include having the
platform meet
the design, construction and use requirements as outlined by American
National Standard
Institute ASME/ANSI B56.6 2002, Safety Standards for Rough Terrain
Forklift
Trucks
sections 5.15.1(a) Use of rough terrain forklifts for elevating personnel
which requires
compliance with Part III, including section 8.24.1(b): Design Requirements
for Platforms
for Elevating Personnel, which states that the Design Requirements for the
Manufacture
of the Platform shall include the following: Paragraph (b) floor
dimensions shall not have
a width greater than the overall width of the truck (measured across the
load bearing tires
plus 10 in. (250 mm) on either side).
Abatement Certification and Abatement Documentation required.
