Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Severe injuries from the misuse of a scissor lift.
a)On May 25 a scissor lift (SL33-015) was being used with the platform
elevated to
approximately 12' above the ground without the the access gate chain in
place, exposing
the operator to a fall from the lift.
Among other methods, feasible and acceptable means to correct these
hazards are to
ensure the scissor lift is operated according to the Operator's manual and
appropriate
sections of the American National Standards for Self-propelled Elevating
Work Platforms
(ANSI A92.6-2006).  Section 7.10.4 states that guardrails shall be
installed and
positioned, and access gates or openings shall be closed per the
manufacturer's
instructions.  Section 8.8.2 states that prior to each elevation the
operator shall ensure
guardrails are installed and access gates or openings are closed per the
manufacturer's
instructions.
