General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of fire and/or explosion.
Employees working in the Material Separation area were exposed to the
potential
for severe burns and/or injuries from a dust explosion which could be
exacerbated
by:
a)The first stage cyclone and second stage dry dust collector which
were located inside the building.
b)The first stage cyclone and second stage dry dust collector were
not fitted with explosion venting.
c)The dust collection system exhaust air was recycled inside the
building.
d)The dust collected in the second stage dry dust collector did not
discharge into a metal container.
e)Local source capture hood connected to a dust collector was not
provided to collect dust produced by the hopper discharge of the
second stage dry dust collector.
Feasible means of abatement include, but are not limited to:
Design, construct and maintain the facility, dust collection system and
shredder
waste system according to the current best practices by establishing
safety, health
and construction standards including, but not limited to:
*National Fire Protection Association (NFPA) 484, "Standard for
Combustible Metals", Chapter 6: Aluminum
*National Fire Protection Association (NFPA) 68, "Standard on
Explosion Protection by Deflagration Venting"
*National Fire Protection Association (NFPA) 69 , "Standard on
Explosion Prevention Systems"
*National Fire Protection Association (NFPA) 77 , "Recommended
Practice on Static Electricity"
Abatement Certification and Abatement Documentation Required.
Submit to the Area Director a written detailed plan of abatement outlining
a schedule for
implementation of engineering measures to control hazards as referenced in
this Citation.
This plan shall include, at a minimum, target dates for engineering
measures which must
be consistent with the abatement dates required by this Citation.
