Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a)On or about 10/29/08 - Warehouse, Shawbox 3 Ton Overhead Crane, hook with a damaged safety latch. Among other methods, one feasible and acceptable method to correct this hazard would be to repair or replace the damaged safety latch in accordance with ASME B30.10, 1993, Section 10- 1.2.3(c).e
