5(a)(1): Section 5 (a)(1) of the Occupational Safety and Health Act of
1970:  The employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm, including severe burns,  to
employees in that employees were
exposed to the hazard of  dust explosions, deflagration or other hazards
from Chrome processing equipment:
a)Processing area was not separated from other parts of the building due
to openings in fire walls which
did not have an approved self-closing fire door.
b)Access doors were not bonded and grounded.
c)A cyclone is not protected with explosion protection systems and is
located inside the building.
d)No devices to limit or prevent explosions in the spray dryer were
provided.
Among other methods, a feasible abatement method to correct this hazard is:
Remove hold open devices on door and repair the latching device and
install doors in other fire wall
penetrations.  Install bonding and grounding wires on all access doors in
equipment.  Relocate cyclone outside
the building and install relief venting or other explosion suppression or
prevention system.  Install devices to
prevent or limit the effects of explosions in the spray dryer.
Installations and modifications  should be done
per requirements of the National Fire Protection Association (NFPA) 654
Standard for the Prevention of Fire
and Dust Explosions from the Manufacturing, Processing and Handling of
Combustible Solids and NFPA 69
Standard on Explosion Prevention Systems and NFPA 68 Guide for Venting
Deflagrations.
