Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: CapStar Drilling LP, Rig 18, Edwards 3 #1, Ector County, TX. For the period of time up to and including 05/09/09, employees were exposed to struck-by hazards due to the vee-slide not being secured to the rig floor. Feasible and Acceptable Abatement Methods would include: 1. Ensure the vee-slide is secured in place prior to leveling the rig. 2. Do not place vee-slide until after the rig has been leveled. 3. Modify the current position of the vee-slide such that it is clearly evident it has been secured.
