and including 05/09/09, employees were exposed to struck-by hazards due to
the vee-slide not
being secured to the rig floor.
Feasible and Acceptable Abatement Methods would include:
1.  Ensure the vee-slide is secured in place prior to leveling the rig.
2.  Do not place vee-slide until after the rig has been leveled.
3.  Modify the current position of the vee-slide such that it is clearly
evident it has been
secured.
