Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Exxcel Well Service, Inc., Rig 15, Andrew Fasken Unit 8 #2, Midland County, TX. For the period of time up to and including 06/24/09, the employer did not protect each employee from caught-between hazards by ensuring employees did not get caught between loads and stationary objects. While unhooking a suspended swivel from a winch, an employee got between the load and a stationary object as the load shifted. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to ensure employees did not get between suspended loads and stationary objects by eliminating the procedure of securing the load to the A-frame and placing the load on the ground.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Exxcel Well Service, Inc., Rig 15, Andrew Fasken Unit 8 #2, Midland County, TX. For the period of time up to and including 06/24/09, the employer did not protect each employee from struck-by hazards by ensuring ground anchors were pull tested prior to the commencement of work. American Petroleum Institute Recommended Practice RP 54, Third Edition, August 1999, Reaffirmed, March 2007, Paragraph 6.3.2 states: "Well operations involving the rig shall not be commenced until the rig is rigged up in a safe manner." Among other methods, one feasible and acceptable abatement method to correct this hazard would be to verify the capacity of each anchor within 24 months prior to commencing work.ncing
