5(a)(1): Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, including injuries resulting from fires or explosions due to a lack of proper well control: No pressure testing was done to assure that the blow out preventer (BOP), when actuated, could hold pressure built up in the well bore in the event that the BOP was actuated. Note:An extended bleed line should be installed so that if the BOP is closed that the pressure can be vented to a safe location away from where employees are working. Among other methods, a feasible abatement method to correct this hazard includes: Provide Blow Out Prevention that will allow for well control to be maintained at all times as per API RP54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, section 6.1.1. Testing procedures for the BOP is outlined in API 53 Recommended Practices for Blowout Prevention Systems for Drilling Wells section 17.3.2. and API Specification 16A Specification for Drill through Equipment section 5.7.3.
