Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a)Warehouse, replenishing side: An employee was lifted to a height of approximately 77 inches while riding on a high lift industrial truck (Crown model 3470F #F13035U). There was no personal fall arrest system in use, on or about 1/15/09. Among others, feasible and acceptable methods to correct this hazard include, but are not limited to the following: 1)Ensure an adequate personal fall arrest system is utilized. 2)Implement precautions for using an industrial truck to elevate personnel as outlined in ASME Standard B56.1-2000 paragraph 4.17.2.
