Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and/or crushing hazards when using homemade lifting devices: (a)Facility, 1000-1100 Maronda Way, Monessen, PA: On or about 1/6/09 employees lifting floor sections (in the facility and in the field) were exposed to struck by and/or crushing hazards when using homemade lifting devices that did not meet the following ASME B30.20-2006 requirements: 1.The rated load shall be legibly marked on the lifting device 2.The lifting devices are to be marked with the manufacturer's information, serial number, lifter weight, rated load, ASME BTH-1 Design Category 3.The lifting device shall be designed by or under the direct supervision of a qualified person and the design is in accordance with ASME BTH-1 4.Initial, interval and periodic inspections of the lifting devices prior to use Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide lifting devices that meet the requirements of ASME B30.20.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential overhead hoist deficiencies: (a)Facility, 1000-1100 Maronda Way, Monessen, PA; On or about 1/6/09 employees who operate overhead hoists in the facility do not conduct frequent inspections to check for any operating deficiencies. Among other methods, one feasible and acceptable abatement method to correct this hazard is to have the operators conduct visual inspections of the equipment in accordance with ASME B30.11.
