Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to falling loads:
(a) Facility, Furnace Plant, Frankfort Road, Monaca, PA:  On or about
November 4, 2008, the two ton hoist
in the furnace plant was being shock loaded and side pulled when picking
up and laying down the drill.
Among other methods, one feasible and acceptable means to correct this
hazard is to eliminate the use of the
hoist for side pulls and to ensure that there is no sudden acceleration or
deceleration of the load resulting in a
shock load on the hoist by complying  with American Society of Mechanical
Engineers, Overhead and Gantry
Cranes (Top Running Bridge, Single Girder, Underhung Hoist) ASME B30.17,
Section 17-3, Operation.
