Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to severe injuries and/or amputations to the arm and/or other extremities: (a)Jobsite, Allegheny Ludlum Bagdad Facility, Leechburg, PA: On or about 6/1/09 an employee who was operating a high pressure vacuum hose was injured when his arm was caught in the open end. The following deficiencies, based on the manufacturer's safety manual instructions, existed in the installation and use of the vacuum line when the accident occurred: (1) The inline vacuum relief valve was not installed within 50 feet from the end of the hose or pipe; (2) The inline vacuum relief valve was used without a lanyard in place and attached to the operator; (3) A bare, open hose end was used during vacuuming; (4) The inline vacuum relief valve was used to shut down the vacuum system when moving the hose and/or piping system; Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the manufacturer's installation, operation and safety instructions as listed on pages including, but not limited to, 8, 15, 21, 22, 23 and 25 of the Safety Manual for Vactor/Guzzler.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being caught in a high pressure vacuum hose: (a)Jobsite, Allegheny Ludlum Bagdad Facility, Leechburg, PA: On or about 6/1/09 an employee who was operating a high pressure vacuum hose was injured when his arm was caught in the open end. The employee had not been trained in the safe operations of the vacuum in accordance with the manufacturer's instruction. Among other methods, one feasible and acceptable abatement method to correct this hazard is to train the employees using the manufacturer's installation, operation and safety instructions as listed on pages including, but not limited to, 5, 7, and 8 of the Safety Manual for Vactor/Guzzler.
