Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by type hazards from an improperly maintained CME-55 drilling rig: (a) Jobsite: On or about September 18, 2009, two employees were operating the CME-55 drilling rig which was deficient in that the: (1) The kill switch was not working; (2) The two hydraulic cylinders on the mast would fall on their own; and (3) There was a missing bolt to an arm on the mast. Among other methods, one feasible and acceptable means of abatement is to comply with the API (American Petroleum Institute) Practice 54, Third Edition, August 1999, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations.l

employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a) Jobsite: On or about September 18, 2009, the Ford F700 drilling rig truck had an obstructed view to the rear and the reverse warning signal alarm on this Ford F700 drilling rig truck was inoperable. In addition, no employee was signaling the driver to proceed in reverse in a safe manner. Among other methods, one feasible and acceptable means of abatement is to repair and/or replace the non working reverse signaling alarm, distinguishable from the surrounding noise levels, and to have an employee signal the driver when it is safe to back up the vehicle.
