employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to an uncontrollable falling load:
(a) Facility, New Castle Road, Butler, PA:  On or about February 3, 2010,
the brakes were not functional on
the bridge of the #1 five ton hoist located at the garage door end. The
hoist continues to travel when the
forward button is released.
Among other methods, one feasible and acceptable means of abatement for
this violation is to ensure that the
bridge brake is in a proper working condition as referenced by the
American Society of Mechanical Engineers,
ASME B30.11, Section 11-1.8.4(d).le

to inadvertent hoist movement:
(a) Facility, New Castle Road, Butler, PA:  On or about February 3, 2010,
the pendant push buttons were not
labeled on the five ton hoist located near the garage door.  In addition,
the bridge was not marked with the
capacity of the hoist on either side.
Among other feasible methods of abatement, one method would be to label
the directional arrows of the
pendant as specified in the American Society of Mechanical Engineers
standards, ASME

there were no frequent (monthly)
inspections being performed, with written reports, on the three five ton
hoists.
(b) Facility, New Castle Road, Butler, PA:  On or about February 3, 2010,
the last periodic (yearly)
inspection performed on the three five ton hoists was performed March 12,
2008.
Among other methods, one feasible and acceptable means of abatement for
this violation is to perform
monthly and yearly inspections in accordance with the manufacturer's
recommendations and the American
Society of Mechanical Engineers, ASME B30.16, Section 16-2.1.2; Section
16-2.1.3; and Section 16-2.3.1.
