(a)  For the employees who work around and under the Bend Pak 2-post
above-ground
lift, Model XP-10ACX, Serial #S175881-00358, where the contact pads on the
lift arms
were damaged or missing, the swing arm restraints were not engaging, and a
lift was not
lowered onto the safety locks when employees were working underneath
raised vehicles,
located at 104 Kasan Avenue, Volga, South Dakota.
(b)  For the employees who work around and under the Grand International
2-post above-
ground lift, Model TPO-9, where the contact pads were damaged or missing,
located at
104 Kasan Avenue, Volga, South Dakota.
(c)  For the employees who work around and under the 2-cylinder in-ground
lift with no
identified rated load capacity and no safety locks, located at 104 Kasan
Avenue, Volga,
South Dakota.
Abatement Note:
Among other methods, one feasible and acceptable method of abatement would
be to
follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which
includes,
among other requirements, the following:
1.Institute a preventive maintenance program according to the
manufacturer's
specifications.
2.Provide training to employees on proper use and care of the lifts.
3.Ensure all lifts are rated and marked with their load capacity.
4.Schedule and complete all periodic and annual inspections.
