Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of: a) Location - 363 Grapevine Road, Wenham, MA: On or about 11/18/08, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) the operator of a Mack dump truck, Model RB688SX, VIN 1M2P271C31M060768 was exposed to the hazard of rollong over of the truck which was placed at the edge of an unstable embankment. Among other methods, one feasible and acceptable method to correct this hazard is to: a) Inspect roadway and shoulder conditions in advance to assure that clearances and road capacities are safe for the passage or placement of any machinery or equipment. b) Ensure that the company training program includes employee recognition of machine safe operation on or within the vicinity of embankments and slopes. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED FOR THIS ITEM.
