Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Being struck by suspended overhead loads. a) Pouring Deck on or about August 25, 2009. - Employees were exposed to being struck by suspended loads as the actuator buttons on the pendant controls of overhead under- slung hoists were not marked as to indicate what direction of travel were control by which button. Among other methods, feasible and acceptable methods to correct the hazardous condition may include: Establish and maintain a monitoring program to ensure that the directional controls on pendants are properly marked to indicate what direction of movement is controlled by each actuator. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the unanticipated restart of equipment. a) Micro Room on or about August 25, 2009. - The drill press in the Micro Area was not equipped with a magnetic anti-restart to prevent the equipment from automatically restarting following any unexpected interruptions in power. b) Pattern Area on or about August 25, 2009. - The drill press in the Pattern Area was not equipped with a magnetic anti-restart to prevent the equipment from automatically restarting following any unexpected interruptions in power. c) Micro Room on or about August 25, 2009. - The polishing/grinding wheel in the Micro Room was not equipped with a magnetic anti-restart to prevent the equipment from automatically restarting following any unexpected interruptions in power. Among other methods, feasible and acceptable methods to correct the hazardous condition may include: the installation of magnetic anti-restart protection devices. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED.
