Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushed by hazards:
a) Worksite is located at 211 W 5th Street, Bayonne, NJ.
An employee was crushed between two 10 yard rolloff containers during the
process of loading a container onto a tilt frame truck.
Violation was disclosed on or about 12/23/08.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is:
1.  The employer follows the mandatory instructions found in ANSI Z245.1
by creating a
written procedure for loading rolloff containers, to include prohibitive
practices.
2.  The employer will ensure all employees are trained on the loading
procedure, including the
prohibitive practices, and keep written training records for such
training.
