Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to struck by hazards:
On or about 22 May, 2009, Employees were exposed to struck by hazards of
the improperly
installed wire rope clips.
1) Clips were not installed based on the size of wire rope. 1 and 2 wire
rope clips were
used on 5/8" wire ropes vice the minimum of 3 clips required by the clip
manufacture.
2) Spacing of wire rope clips was not of a uniform distance when 3 clips
were used.
3) An improper amount of rope was turned back for rope termination.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Use proper rigging techniques for use of wire rope
clips as
outlined in 1926.251 Table H-20
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.

On or about the 22 of May, 2009, the employer did not protect the derrick
hand from the
recognized hazard of falling from an elevation in that he allowed the
employee to ascend the
derrick structure without personal protective equipment in the form of a
Personal Fall Arrest
system.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: To ensure employees climbing the derrick wear a
Personal Fall
Arrest system as required in API 54 5.5.1
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
