Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling hazards: The climbing assist 5/8 inch wire rope was installed with two clips. Employees were exposed to hazards associated with two wire rope clips failing, causing the employee to fall from the derrick mast to the rig floor below. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Install three wire rope clips as recommended by the Wire Rope Technical Board as referenced in the Wire Rope Users Manual, Chapter 4, Table 6 or 7. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
