Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to serious burns from potential dust deflagration, explosion,
or other fire hazards:
The following conditions found at the facility increase the potential for
an explosion and/or
potentially increase the severity of an explosion:
a.A means of static charge dissipation, such as bonding and grounding, was
not provided
within equipment containing wood dust, between equipment and the exhaust
ventilation
system, and between segments of the exhaust ventilation system, including
but not limited
to the wide belt sander, the small sander, the saws, the jointer, the
shaper, the planer,
and the plunge router.
b.Use of flexible non-metallic duct work exceeded the minimum required for
connection
of the metallic ductwork to equipment, at equipment including but not
limited to the
downdraft table where handheld sanders are used.
c.Deflagration isolation or suppression systems were not provided for the
inlet (dirty air)
sides of the ventilation system.
d.The dust collectors located outside the building did not have adequate
means of
mitigating a deflagration.
e.The recycled exhaust air from the dust collectors did not have a
high-speed abort gate
activated by a spark detector.
Among other methods, one feasible and acceptable method to correct this
hazard is to have a
process hazard analysis conducted by an individual knowledgeable about
combustible wood dust
and comply with applicable standards issued by the National Fire
Protection
Association
(NFPA), including NFPA 664, Standard for the Prevention of Fires and
Explosions in Wood
Processing and Woodworking Facilities.
