A.  Employer knew or should have known that the driver not being at the
controls of a Lull
644E-42 Model forklift while 2 employees were on a  forklift elevated
platform, in excess of
16 feet high at back side of building, constituted a hazard the employees
having no safe
means to get down in an emergency.  A useful and feasible method, among
others, to correct
the hazard is to have the driver stay at the controls when employees are
on the platform.
