SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employee knew or should have known that an employee elevated on a personal platform of a (GEHL) Model 553 forklift in excess of 12 feet high with no operator at the controls was exposed to the hazard of no means to egress from the platform in event of an emergency at the AT&T telephone job site. A feasible and useful method among other methods is to ensure that an operator is at the controls while an employee is elevated on personal platform.
