A.  Employer knew or should have known that operating a Bobcat forklift,

inoperable reverse signal alarm and no observer used constituted a hazard
of employees being
struck by forklift when operated in reverse gear at the Fort Mill
Elementary School Number
7 job site.  A feasible and useful method, among others, to correct this
hazard is to repair
the reverse signal alarm.
