Clark LP forklifts in the establishment were exposed to the hazard of
being thrown from the
forklifts and crushed due to the forklifts not being provided with seat
belts.  A feasible and
useful means of abatement, among other methods, is to provide and require
the use of seat
belts by employees operating the forklifts.
