A.  Employer knew or should have known that operating a Lull forklift,
Model 844C-42,
with inoperable reverse alarm and no observer used, constituted a hazard
of employees being
struck by the forklift when it is being operated in reverse gear at the
Buckwalter Community
Center job site.  A feasible and useful method, among others, to correct
this hazard is to
repair reverse signal alarm.
