A. Employer knew or should have known that employees operating the
forklift without
wearing a seatbelt were exposed to the hazard of falling out of the lift
and being crushed
beneath it.  A feasible and useful method of abatement, among others, is
to require and
enforce the use of wearing a seatbelt when operating forklifts at the
facility.
