SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that an employee operating the Hyster #45, Serial number 3010809317P, without wearing a seatbelt was exposed to the hazard of being thrown from the lift and crushed by it. A feasible and useful method to correct this hazard, among others, is to enforce the use of the seatbelt while operating the forklift.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that two employees operating the Selco Baler, Serial number 01871366 without the use of a safety interlock or a constant pressure switch were exposed to the hazard of having their hand crushed by the compacting ram head. A feasible and useful method to correct this hazard, among others, is the installation of a working safety gate interlock and the constant pressure switch. B. Employer knew or should have known that two employees operating the McDonald Baler, Serial number unknown, without the use of a safety interlock or a constant pressure switch were exposed to the hazard of having their hand crushed by compacting ram head. A feasible and useful method to correct this, amongst others, is the installation of working safety gate interlock and the constant pressure switch.
