employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of being stuck by plastic shrapnel associated
with the rupture of
polyvinyl chloride (PVC) plastic pipe:
a)  At the site, through out the plant on the lower level, schedule 40 PVC
pipe was being
used to carry 120 psi of compressed air throughout the plant, where
employees were using
compressed air for cleaning and machine operation.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to encase the PVC pipe in conduit or shatter-resistant material
or replace with metal
(copper or steel) pipe.  (Reference Plastic Institute Recommendation B
"Thermoplastic Piping
for the Transport of Compressed Air or Other Compressed Gases, 2005").
