on the
rear of the packer panel, such as "DANGER - STAND CLEAR WHEN
PACKER PANEL IS IN MOTION", on or about 01/26/2009.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method
to correct this hazard is to place a sign on the packer panel that states
"DANGER - STAND
CLEAR WHEN PACKER PANEL IS IN MOTION" as described in ANSI Z245.1-1984
Section
7.2.2.5.1 (Rear Loading Equipment, Safety Signs).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falling off the riding step of a moving refuse truck:
a)256 Forest Avenue, Lyndhurst, NJ:  Truck #21 did not have caution signs
affixed
to the sides of the vehicle above the riding steps and at the driver's
station, such
as "CAUTION - DO NOT USE RIDING STEP WHEN THE VEHICLE IS
EXCEEDING 10 MPH OR OPERATING IN REVERSE", on or about
01/26/2009.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to correct
this hazard is to affix caution signs to the sides of the vehicle above
the riding steps and at the
driver's station stating "CAUTION - DO NOT USE RIDING STEPS WHEN THE
VEHICLE
IS EXCEEDING 10 MPH OR OPERATING IN REVERSE" as described in ANSI Z245.1
Section 7.2.2.5.2 (Rear Loading Equipment, Safety Signs).
