Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards, caught between, and struck-by hazards caused
by the overloading
and subsequent collapse of parallel chord roof trusses:
a)20 Farley Road, Brunswick, Maine - On 11/13/08 a stack of 4 foot by 8
foot by
3/4 inch Advantec building material was placed on parallel chord, metal
plate
connected  wood roof trusses that had been set and braced.  The stack was
placed
at the north end of the building near the center of the trusses' span.
The height
of the stack was approximately 33.75 inches, exceeding the maximum
sheathing
stack height of 16 inches specified by BCSI-B4 Summary Sheet established
by the
Wood Truss Council of America (WTCA), representing the Structural Building
Components Industry.  The excessive stack height resulted in a stack
weight of
approximately 3546 pounds.  Upon releasing the stack from the crane
slings, the
trusses collapsed and the two employees fell approximately 17 feet to the
ground
below.
b)20 Farley Road, Brunswick, Maine - On 11/13/08 a stack of 4 foot by 8
foot by

placed
at the southwest corner of the building.  The height of the stack was
approximately 33.75 inches, exceeding the maximum sheathing stack height

Council of America (WTCA), representing the Structural Building Components
Industry.  The excessive stack height resulted in a stack weight of
approximately
3546 pounds.
Note:  Feasible and acceptable abatement methods to correct this hazard
would be to
follow the Wood Truss Council of America's BCSI-B4 Summary Sheet that was
provided
to Bilodeau Drywall, the truss installer, upon receipt of the trusses at
the construction
site.  The summary sheet specifies precautions to be taken to prevent
overloading parallel
chord, metal plate connected wood trusses.
