Section 5(a)(1) of the Occupational Safety and Health Act 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or physical harm to employees in that employees were exposed to the hazards of being struck by an unsecured compressed gas cylinder: On or about March 23, 2009, at the site of OSF Saint Francis Medical Center, employees of Pipco Companies, LTD., were exposed to the hazard of being struck by an unsecured Nitrogen compressed gas cylinder. The cylinder, with the regulator attached, was being transported on a standard two-wheel dolly, not designed for transporting or securing compressed gas cylinders. Among other methods, feasible and acceptable abatement are to: 1. follow the Compressed Gas Association Pamphlet P-1-1965, in which 3.4.4 states before using cylinders, be sure it is properly supported to prevent it from being knocked over. 2. Use a suitable cylinder cart or dolly, fastener or steadying method to keep cylinders from being knocked over. Disclaimer: 1. You are not limited to the abatement methods suggested above. 2. The methods explained are general and may not be effective in all case. 3. The employer is responsible for selection and carrying out an appropriate abatement method.
