Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees loading P88731NAT 51" paper on the Reelite 1 ton hoist system located on the Paper Compactor machine were exposed to crush-by hazards and the employer did not ensure the hoist was not loaded in excess of its rated load. ASME B30.16-2007 Overhead Hoists (Underhung): Section 16-3.2.1: "The hoist shall not be loaded in excess of its rated load except for test purposes as provided in paragraph 16-2.2.2 or for planned engineered lifts as provided in paragraph 16-3.2.2" Among other methods, feasible and acceptable abatement methods to correct this
