a.At 4525 Dean Martin Drive, Las Vegas, NV an employee cleaned windows from
a window washer stack ladder at a height of approximately 30 feet. Work
began
approximately 10 feet from the 41" guardrail on the 25th floor balcony. The
employee was exposed to potential serious injury in the event that the
ladder
failed, causing him to fall approximately 30 feet to the concrete surface.
