Nevada Revised Statute 618.375(1):  Duties of employers.  The employer
failed to furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm to his employees:
The employer did not ensure that employees were protected from unsafe
working conditions as
follows:
a) At Building #21, three employees were using the Miller RA15-1 roof
anchor that was
visibly damaged and was installed incorrectly, exposing them to a fall
hazard of
approximately 30 feet due to failure.
Reference:  Miller RA15-1 Roof Brackets Installation Manual:
1. Section II. Installation, item B: "Refer to Figure 1 for proper setup"
2. Section II. Installation, item C: "Note: The RA15-1 Roof bracket is
reusable
provided it has not seen fall arresting forces and has not been damaged in
any
way. Inspect the Roof Bracket and all components before each use. The
bracket
must not show any signs of distortion or tearing."
One feasible abatement method would be to follow the Miller Installation
Manual for proper
installation of the RA15-1 roof brackets.
