employment and a place of
employment which was free from recognized hazards that were causing or
were likely to cause
death or serious physical harm to his employees:
The employer did not ensure that forklifts were used in accordance with
manufacturer's
specifications, as follows:
a.At 5000 S Decatur Blvd., Las Vegas, NV, Kwong Yet Lung Co., Inc.
employees
were using one Hyster and 3 Yale forklifts with a wooden pallet to elevate
personnel to heights of 4 to 12 feet above a concrete floor.  The
manufacturers'
operater manual specifically warned against elevating workers on the
forklifts
unless an ANSI B56.1 compliant work platform was used.  Employees elevated
on the forklift contrary to manufacturer's warnings were exposed to
serious fall
injuries.
