Nevada Revised Statute 618.375(1): Every employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees: The employer did not ensure that the required Operator's and Owner's Manual was available to authorized powered industrial truck operators, as follows: a.At Hansen Mechanical Contractors, Inc., 6285 South Valley View Boulevard, Suite A, Las Vegas, NV 89118, authorized powered industrial truck operators were operating a forklift (make: Toyota, model no: 42-5FG25, serial no: 405FGU25-76781) that did not have the required operator's and owner's manual. The operators had been leaving the forklift unattended on an inclined ramp/driveway without blocking the wheels. Reference: Toyota Operator's and Owner's Manual (5FG10-30, 5FD10-30), Cover. Warning! *Make sure these important instructions are available for the operator. Keep this manual in the pocket on the seat back. *Do not start the truck unless you understand these instructions. One feasible means of abatement would be to keep a back up copy of the correct operator's and owner's manual on hand to replace the original in case it becomes misplaced to ensure these important instructions are available to forklift operators at all times.
