The violation most recently occurred at the employer's facility located at 2401 5th Ave S, Texas City, TX on or about September 21, 2010 in the Pipestills 3B Unit, where pipefitters performing maintenance on the heat exchanger 427-C. Employees were exposed steam at high temperatures and struck by hazards.ures
