Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that the employees were
exposed to the hazard of falling:
a/  At the job site, the wooden, job-made ladder used by employees to gain
access to the working platform
supported by a tubular weld walkthrough frame scaffold was not equipped
with filler blocks in-between all 13
cleats.
One method of abatement to correct this apparent hazard is to install
filler blocks in-between each cleat as
required by ANSI A14.4-1973, paragraph 4.5.2.
