Carolina:  The employer did not furnish employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fall hazards:
a)feed mill, operators remove various bin covers to check the grain level
in the
silos by lowering a weight attached to a tape measure.  When the bin cover
is
removed, the operator is exposed to a fall of up to 60 feet through the
unguarded
24 inch diameter hole.
One feasible method of abatement would be to install a capped 6 inch
diameter
port on the top of each bin cover.
