Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing/struck-by hazards:
(a) In the service department of Clark Community Oil for employees working
around and
under the Challenger lift, model #24012XL, serial #030100000002184, with
excessive
oil and grease on the lifting chains and cables and the Rotary in-ground
hydraulic hoist
that had damaged support pads, an annual inspection and employee training
had not been
conducted.
Abatement Note:
Among other methods, one feasible and acceptable method of abatement would
be to
follow the requirements of ANSI/ALI ALOIM and/or ANSI B153.1 which
includes,
among other requirements, the following:
1.Institute a preventive maintenance program according to the
manufacturer's
specifications.
2.Provide training to employees on proper use and care of the lifts.
3.Ensure all lifts are rated and marked with their load capacity.
4.Schedule and complete all periodic and annual inspections.
