Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing and struck by hazards from the improper use of an
aerial lift:
a) Work Site - On or about February 10, 2009, a Genie S-60 Aerial Lift,
serial number
2292, was being utilized to lift a light post suspended from the work
platform of an aerial
lift not rated for lifting overhanging loads.
ABATEMENT NOTE: One feasible means of abatement is to employ the use of
material
handling equipment designed with the intent to hoist and place loads. This
violation finds the
aerial lift was employed in a manner not permitted by the Genie S-60
Operator's Manual as the
aerial lift was not rated for the lifting of overhanging loads.  ANSI
A92.5-1980, American
National Standard for Boom-Supported Elevating Work Platform Section 13.2
During Operation:
requires use of work platform in accordance with the manufactures/owner's
operating
instructions and safety rules.
No abatement certification or documentation required. Improper usage of
the Aerial Lift was
halted during the inspection.
