Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a serious fall, crush and amputation hazard while being lifted on forks of industrial trucks: a) Warehouse - Employees were being lifted on the forks of the industrial trucks to access the racks up to 136 inches high, on or about April 13, 2009. ABATEMENT NOTE: One feasible and acceptable method of abatement would be to elevate personnel on a safety platform, attached to the forks, which was approved by the manufacturer in accordance with ANSI B56.1-2005 Section 7.37, Safety Standard for Low Lift and High Lift Trucks. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.tives
