Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by and crushing hazards:
a) Building exterior - On or about 23 November 2009 employees worked from
a JLG600S boom lift without unloading it from the trailer, exposing them to
potential injury in the event of a tip-over.
ABATEMENT NOTE: Among other methods, feasible and acceptable abatement
methods to
correct this hazard :
1) Comply with manufacturer recommendations in the operator manual on page
1-2 "Do not operate the basket while on trucks, trailers..." unless
approved in
writing by JLG.
2) Comply with ANSI A92.5, paragraph 7.11.12 stating "aerial platform
shall not
be operated while on trucks, trailers, railway cars, floating vessels,
scaffolds, or
similar equipment unless the application is approved in writing by the
manufacturer".
3) Unload the lift prior to using it.
Abatement certification required within 10 days after abatement date.  The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.tives
